The Registered Practical Nurse (RPN) is valued member of an inter-professional Mobile Response Team (MRT) which is comprised of RN/RPNs and PSWs. The MRT provides direct care, intervention, and support to older adults living in a long-term care home (LTCH) within a geographic region. Clients include those who are demonstrating responsive behaviours associated with the various types of dementia, delirium, late onset mental health, addictions, or other neurological conditions.
Working in collaboration with the MRT Registered Nurse and the Personal Support Worker, the MRT RPN will enhance the quality of life for referred clients by assisting with transitions, providing direct care and support, and identifying triggers and successful strategies for responsive behaviours. The MRT shares in the care of the resident with the LTCH and will build capacity through assisting and modeling person centred care strategies as well as other knowledge exchange activities. The RPN’s work is conducted in a manner that is consistent with the Mission, Vision and Values of Providence Care Centre.
Education & Experience
- Current unrestricted registration as an RPN with the College of Nurses of Ontario
- RPN Diploma (with medication certificate or full scope of practice) from an accredited college
- Current CPR and First Aid Certificates
- Minimum one year of related experience in providing support to older adults with responsive behaviours related to dementia, mental illness, addictions, or other neurological conditions. Three years preferred.
- Experience working in various care settings caring for older adults who demonstrate responsive behaviours in conjunction with complex cognitive-mental health and/or physical care needs.
- Experience in the supportive management of responsive behaviours
- P.I.E.C.E.S. TM, Gentle Persuasive Approaches, U-First! and DementiAbility training an asset
- Certificate in dementia care (or equivalent) from an accredited educational institution, preferred
- Strong written and oral communication skills
- Experience with quality-improvement processes an asset
- Prior experience working in collaborative practice settings an asset
- Ability to work effectively with others on an inter-professional care team
- Satisfactory criminal-record and vulnerable-sector screening
Position Duties & Responsibilities
- Provides direct care to clients in collaboration with the MRT RN and PSWs as well as with the client’s LTCH team, and family care partners during transitions in care and/or when behavioural support is required
- Develops assignments and provides support for MRT PSWs through collaboration
- Works collaboratively with other Seniors/Geriatric Mental Health Behavioural Support Services including Seniors/Geriatric Mental Health Outreach Teams and Psychogeriatric Resource Consultants.
- Utilizes evidence-based screening, observation, and assessment tools during assessment with referred clients, family members, LTCH team, and other health care system partners as required;
- Supports clients in the development and implementation of a Collaborative Care Plan tailored to the client’s unique needs in order to improve quality of life and assist in the supportive management of responsive behaviours
- Ability to work in all LTCHs within the assigned region
